If you live within our practice area you are welcome to register with us.
Our reception staff will be happy to guide you through the procedure.
If you live outside of our practice area, we would be able to register you but as an ‘out of area patient’. This entitles you to access to all our services, except for home visits and you will need to sign a form stating you agree to those terms. This will not be a suitable arrangement for you if you would not be able to come in to the surgery.
New patients will need to complete a registration form (GMS1) and a health questionnaire, which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. We also require one form of photographic ID when you hand the forms in. GMS1 registration forms are available from our Reception Team by calling 01229 402900 or by downloading the GMS1 Form here.
All new patients between the ages of 40 and 74 will be oﬀered a health check with a member of the healthcare team at some stage following registration to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Medical treatment is available from the date of registration. Please contact reception for further information.
Every patient is allocated a named or usual GP when they register with us or that is requested either by the patient or a specific GP who normally looks after a patient. If you would like to know which GP is your usual GP please ask us when you contact us.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice as a temporary resident.
You can be registered as a temporary resident for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. You cannot register as a temporary patient at a practice in the town or area where you are already registered. You can download the ‘Temporary services form GMS3’ from the NHS UK website, for temporary registration.
